This tool allows for designing guidelines for the creation of prototype design ideas. The sheet defines the steps to be followed to obtain a prototype that represents a more or less tangible component of a service to create an initial experiential form of the service. Filling out the sheet helps the working group in designing the prototype.

Instructions for compilation
1. Indicate the title of the project.
2. Define the question that drives prototyping.
3. Define the object of prototyping, which may refer to an entire service or a part of it. It may be an experience or process, physical objects and products, environments or spaces, software or digital products, or systems.
4. State the resources available for prototyping. These include: the economic resources, materials, places, people and known data of the project being prototyped.
5. Structure the actions of each stage of prototype construction. At this point keep in mind:
- The choice of context, actors, techniques
- How to use the materials chosen and at what level of fidelity and detail to develop the prototype
- What tools to use for data collection (e.g., photographs, videos)
- How to collect, store, and analyze data and feedback.
6. Define the desired results based on the demand and purpose of prototyping.
